SPECIFICATIONS VOUCHER
SERVICES Any
exception to these Service Specifications must be requested in advance in
writing and approved by the Deputy Administrator. SERVICE
DEFINITION: A voucher service program subsidizes the cost of an essential service
provided to an individual. A voucher service program subsidizes the cost of an essential service
provided to an individual 60 years or older, or as eligible age is defined by
the funding sources. GENERAL
REQUIREMENTS: Grantees are required to
meet all applicable Federal and State laws related to taxation, labor and
related law. SERVICE
CATEGORIES AND UNIT MEASURES: The
Service Specification for Voucher Services covers programs funded
by the Administration on Aging (AoA), the State of Nevada, Independent Living
Grants, the Alzheimer’s Project and any other funding source where the
grantee acts primarily as a fiscal agent. Reporting is based on the
funding source requirements. The definition of Units of
Service includes but is not limited to the following voucher programs: Taxi Voucher
Programs:
Individuals 60 years or older may purchase taxi vouchers at half the
retail value. One coupon book, valued at $20.00 of taxicab
fare or tip, may be purchased from the grantee for $10.00. Participating taxi
companies receive reimbursement from the grantee for the full value of the
coupon, one half from grant funds and one half from fees paid by the
client. Taxi Voucher
Programs: One unit equals the distribution of one coupon book (20
vouchers). Transportation Tokens:
Individuals 60 years or older may receive transportation tokens as needed
and determined by the granting agency. Transportation
Tokens: One token equals one unit. Respite Voucher
Programs:
Respite voucher certificates valued up to $1,000 annually may be issued
to a designated caregiver to provide supervision of an eligible individual 60
years or older, or of a different age as defined by the funding source. For
example, the federally funded Alzheimer’s Project permits service to
individuals under the age of 60 years of age. The grantee may establish
agreements with facilities and agencies willing to accept a voucher for services
and bill the program directly. In the event that respite expenses exceed the
value of the certificate, a caregiver may supplement the voucher with personal
funds. Any expense over an award amount is the caregiver’s financial
responsibility. The maximum award possible is $1,000. A caregiver may select a
private party or agency, other than him/her self, or agency to provide
supervision. The provider
agrees to submit a signed statement of services provided and to accept an
established reimbursement rate (i.e. minimum wage). A signed statement that
includes service hours must be submitted to the program in order for
reimbursement to be made. Respite Voucher
Programs: One unit equals one hour
of respite care regardless of the cost to provide that unit. Programs may issue
voucher certificates valued up to $1,000 per caregiver annually. (A caregiver
may request coverage for a second senior living in the same home.)
SPECIFICATIONS--ELIGIBILITY: In accordance with funding
source requirements, there may be variability among the definition of persons
eligible for service as well as a difference in the amount of financial support
that may be provided. 1.
Eligibility – All voucher programs: 1.1
Any individual who is 60 years or older, or as age is defined by the
funding source. 1.2
Any senior citizen who is a State of Nevada resident. 1.2.1
Residency may be established by a Nevada driver’s license, a State of
Nevada identification card, a rental or utility bill, a verification of voter’s
registration or some other official document. 2.
Eligibility
– Taxi Voucher Programs 2.1
Any individual 60 years or older. 2.2
Depending upon the funding source, residency may be required within a specific
county. (See: Notice of Grant Award and Service Specifications General
Requirements or Service Specifications to determine funding source.) 3.
Eligibility – Respite
Voucher Programs 3.1
An individual 60 years or older
who is a State of Nevada resident who receives live-in supervision/care by a
family member or a non- primary caregiver (e.g. live-in friend). 3.1.1
The need for supervision has been verified by a physician, a social or
health agency, the program staff or some other method of verification that has
been approved by the Division. OTHER
SPECIFICATIONS: 4.
Expiration dates for all voucher certificates/books must be approved by
the Division. 4.1
Voucher books for taxi programs may have up to a 6-month expiration date
from the end of the funding period. 4.2
Voucher certificates for respite programs may have up to a 3-month
expiration date from the end of the funding period. 4.3
Transportation tokens may be used as needed by the client. 5. Reimbursement and Income
Eligibility Schedules (where applicable): 5.1
Programs may establish a reimbursement
schedule to evaluate the income and/or liabilities of the senior recipient in
order to determine the amount of funds for which a caregiver is eligible. 5.1.1
Reimbursement schedules must be reviewed by the Division 5.2
A self-declaration form or other method of determining family income
and/or liabilities will be used to determine level of eligibility. 5.2.1
The program may consider large obligations (i.e. medical expenses) in
evaluating reimbursement eligibility. 5.2.2
Random audits may be conducted to verify eligibility. 6. Operating Procedures and
Documentation Requirements: 6.1
Residency for the senior and the caregiver can be established by a Nevada
driver’s license or other legal document, such as a State of Nevada
identification card, a rental or utility bill, verification of voter’s
registration or some other official documentation. 6.1.1
Residency verification by a physician’s office, a social or health
agency is acceptable. 6.2
Taxi Voucher Programs will develop and maintain: 6.2.1
Numbered vouchers for tracking purposes. 6.2.2
A voucher log (e.g., voucher distribution to clients, taxi company
reimbursement). 6.2.3
An application that will include required client information. 6.2.4
A process for reconciling information on the person who purchases
vouchers, how many vouchers are purchased and the monthly usage. 6.2.5
A report on the total number of vouchers that have expired or are
unused. 6.3
Respite Voucher Programs will develop and maintain: 6.3.1
A referral log of agencies willing to accept and bill for services. 6.3.2
A respite voucher application that will include Caregiver and Care
Receiver information. 6.3.3
A process for the documentation of the number of hours of service
provided by the respite facility or independent worker (e.g. timesheet, vendor
invoice). 6.3.4
Programs may require recipients to use a portion
of the award within the first 90 days 6.3.5
A process for reconciling information on the person who purchases
vouchers, how many vouchers are purchased and the monthly usage. 6.4
Transportation Token Programs will develop and maintain: 6.4.1
A token log (e.g. Receipt of
tokens from transportation provider, token distribution to clients.) 6.4.2
An application that will include required client information. 6.4.3
A process for reconciling information on the person who receives tokens,
how many tokens are distributed and monthly usage. 6.4.4
A report on the number of tokens received/purchased from the
transportation provider and the number of tokens distributed to clients. 7.
Waiting Lists – All Voucher Programs 7.1
A waiting list will be maintained in the event the demand for service exceeds
the program’s capacity to provide service or to meet the demand. 7.1.1
Waiting list documentation must include at a minimum: ·
The client’s name, address, and
telephone number. ·
The date the client was placed on
the waiting list. ·
A procedure, where feasible, to
prioritize service according to the urgency of a client’s situation. ·
A procedure for updating the
continued service needs of clients placed on the waiting list. 8.
Quality Improvement: 8.1
Client Satisfaction and Performance Indicator measures are required by
programs. 8.1.1
Performance
indicator measures will be completed on all clients new to the service and
follow up surveys will be done within six months of the issue date of the
certificate. ·
Where a large number of new clients are enrolled, a client sample may be
used as approved by the Division.
8.1.2 Performance
Indicator questions must be approved by the Division. 8.1.3 The tool used to implement Client Satisfaction and Performance Indicator surveys may be combined whenever appropriate.
